I have a Physical RFC that needs to be completed. Has anyone else had to get one filled out and have you been successful doing so??

My neurosurgeon completed mine (I mean his nurse) and my lawyer and SS has copies of it. There is one on the SS website, but I used one that is used for Chiari/Syringomyelia...

Its good to have your main treating doctor, or specialist completing the form.

Thats my understanding...if someone else has info in regards to the Residual Functioning Form please share!

My urologist is the one who completed mine. I would suggest you take it to your doctor that is treating you for your disabling condition. Good luck to you and take care.

I had my PCP to fill one out for me although I never had a chance to give it to SSD. I used the one off of SS website and I took it to my PCP, in case SSD ever ask for one. Good luck. Brian

took it to my urologist at the hospital and got it done

I was actually just looking at my file from the OHA. I never went to a hearing because in a pre-trial review, the judge gave me an on the record Fully Favorable Decision. What was disturbing, when looking through my file. My PCP and my NS put together very nice RFC's, but there is no evidence that the judge took either one into consideration. The State "behind the desk" Dr. did an RFC based only on my records and the state orthopedic examiner SSA who did an exam on me did an RFC. The "behind the desk" Dr. put together the most ridiculous RFC I could imagine, said I could lift over 20 lbs and walk or stand 6 or 8 hours a day. (which is impossible) The Dr. I was sent to for an exam barely looked at me, but his RFC was closer to what my own Dr's must of put down. The judge, in his decision, based his favorable decision on what the actual examing Dr. opinion was since he actually did an examination.
The bottom line is, I was approved for SSA, but knowing that my future lay in the hands of the opinion of that examing Dr is nothing short of scarey. He barely looked at me, or my file, refused to look at the MRI's I took him and he was extremely grumpy. If I find that at review time, I'm faced with this guy again, I'm really going to be nervous.
