My problems started in 2000-knee problems,fibro, OA. went whole route-p.t.,orthopedist-torn meniscus surgery,rheumatologist,acupuncture,chiropractor,ma ssages. Nsaids not much help. Tramadol somewhat helpful but not lately. Fibro was better for while but now back again. Knees,hands, neck better but now lower back-OA? Should I see a pain doc or back to rheumatologist? Thanks. Only you can really answer that question....something I always tell "newbies" (and you may not be new to pain)....ask yourself if you function far less now...if so, and could get better relief would you function better? If your answer is that you would function better getting better relief then a pain specialist is where I'd be heading.
It sounds like you've done the various doc route....we all have. If you get a little relief from Tramadol then something a touch stronger might be what you need....however, only you can truly know that. Personally, if I were in your position, I'd be asking for a pm doc. There are also many many different non-med things that can be offered to you through a pain doc. You may get enough relief from blocks that you don't need to take pain meds...maybe not...
A particular med I'd be asking for if I were you (fibro and all) is Lyrica. I know that it helps me more than anything....it's tough to get used to at first....you have to start low and titrate higher slowly...the side effects are basically like being drunk...but you get used to that over time and it does go away. That is something you could try right now while you wait on a pain doc...keep us informed of what you decide....

I agree with Troy that I would find a PM to deal with your pain. Part of that is to refer you to a doctor that could treat the source of your pain.
