See if any of these sites will help you with your HMO (they are specific to your state):
Monitoring the Pulse of the State’s Top HMO Plans Our Annual Survey (http://www.calbrokermag.com/Magazine/story/oct07/hmosurvey.htm)
DEPARTMENT OF MANAGED HEALTH CARE CALIFORNIA HMO HELP CENTER DIVISION OF PLAN SURVEYS (http://www.hmohelp.ca.gov/library/reports/med_survey/parity/055.pdf)
Health Administration Responsibility Project Tort Theories of HMO liability (http://www.harp.org/tort.htm)
I also just "realized" that you're in CA, so it's likely that Dr. Schievink has 'dealt' with your insurance company before. Remember, he told me that Cedars will work with patients and insurance companies that are reluctant to 'permit' coverage. Seems like you are too close to the 'expert' to let an opportunity like this pass you by!
