i am trying to find out what the equivilant of 40 mg. of methadone is for oxycodone. And also morphine.
which would be the best to move to???

Simby, I don't have a conversion for you but do l recommend you talk to your doctor and follow his directions as the half-life for methadone is tricky and it would be easy to overdose if you do it without help. Trying to find the conversion chart is a good start but what is the change over levels so you don't risk death.

This website should give you an approximate conversion. Shoot, I can't get it to copy and paste. I posted it in useful websites. It's globalpharm....something.
I am getting off methadone also but a much smaller dose. I've been on 12.5 mg methadone and 16 mg dilaudid for a long time but always thought the methadone was making me sick. Long story but.....it was and is. The conversion at my dose was equal dilaudid and methadone. So.....I stopped the methadone cold turkey and covered with same equivalent with Diliaudid. It's only been a week but I'm still dealing with withdrawal. My Dr. ordered a titration of ....drop 2.5mg, 1 week, drop 2.5 second week, drop 2.5 third week and then stop at 4th week. I opted to just stop as I want this overwith asap. Again.....my dose is MUCH smaller so don't count on a smooth conversion with equal conversion (in my experience).
Edit to add: the above methadone taper was along with increase in my diluadid to 28 mgs day to take at the same time. Hope this makes sense. My mind has been pretty fuzzy for quite a while now.
Good Luck! If I remember correctly, you are extremely med sensitive too.

My PM has told me Methadone has a longer half life than some opiates. With that in mind you really do need to work with your doc to titrate the doses while he adjusts you to another med. I din't think this is something you should do alone. Just my opinion.

thank you all for replying. I misunderstood my friends request. She want to change from 100 mg. methadone a day to something else.
i told her that her doc is the one to ask. So that is what she is doing today.

i am one of the lucky ones who manages the pain with vicodin and advil and flexaryl.
