Virtually all the topical patches release medication when they are placed on the body and "warm up".. a body temp above 101-102 will do one of two things .. increase the medication release and/or degrade the drug. Also increasing body temp will cause perspiration .. which often causes the patches to come loose

Steve - is it ok to wear the patches under a thermacare? just for the benefit of the lidoderm and heat to loosen the muscles?

I know that your question is directed for Steve to respond, only... I am going to jump on in and say: I would not do it. Not with Thermacare, Icy Hot, Deep Heat.
I have the Lidoderm patches, also. I made the mistake of using another "heat" source, such as one you're asking about.
It felt as if my skin was lit on fire.
After I had made the mistake, my Rx'ing doc told me, not to do it. Duh! I found that out the hard way.
It really hurt!
Natural heat? from sun? That is not an immediate and direct, cutaneous application.
I would not use a topical along with Lidoderm.
